Digital Forms

Overview

Every clinic and hospital has its own paperwork — procedure consent forms, screening questionnaires, aesthetic assessments, medication receipts, medical certificates, pre-operative checklists. General hospital software does not ship with these, and having each one custom-built is expensive and slow.

Digital Forms lets a facility convert its own paper into digital forms without help: upload the existing PDF, drag fields onto it where the blanks are, publish, and start using it. No code, no waiting for a vendor.

Why it matters

Converting paper solves several problems at once:

• No more double entry — the patient fills it in once on a tablet and the data is in the system
• Findable — where did that consent form go? Search by hospital number and it is there
• Legally usable digital signatures — captured with a hash and timestamp, verifiable afterwards
• Update forms yourself — when a regulator revises a form, an administrator changes it in five minutes
• No paper storage costs — digital forms do not get wet, torn or lost

Three steps to use it

1. Upload the background

Upload a PDF or image of the existing document. The system stores it as an asset and displays it behind the form.

2. Place the fields

In the builder, drag the field types you need — text, checkbox, signature and so on — onto the positions to be filled. Resize them, and add validation, default values or a required flag.

3. Publish and use

Publish, and the form is immediately available. Create an instance against a patient, hand the tablet over or have staff complete it, then finalise — which locks the form and produces the PDF.

Proper version control

Forms used with real patients need care: editing one midway would otherwise affect instances already completed.

MorLuang separates three things:

• Template — one kind of form, such as “laser treatment consent”
• Version — v1, v2, v3 of that form
• Instance — a completed form, bound to the exact version used at the time

Edit the form and you create v2. New instances use v2; existing ones stay on v1, untouched. An audit later shows exactly which version was in force.

How this differs from a general form builder

Unlike general-purpose tools that only produce simple forms, this one is built for healthcare:

• Bound to a patient and visit automatically — no re-keying the hospital number
• Base details auto-filled — name, age and sex pulled from the record
• PDF export matching the original layout — printed output looks like the paper document, which matters for legal records
• Signature hash — a cryptographic hash accompanies each signature as evidence
• Field-level audit log — every change recorded with who and when
• Embed mode — attach a form to another workflow, so opening a visit prompts the consent form
• Multi-tenant — each facility’s forms are its own, never shared

Digital Forms works with the outpatient module (a visit opens and the required form appears), the inpatient module (admission forms and procedure consent), the medical record (auto-filling details and filing the result as a note), and billing (facility-specific receipt layouts).
